Well, it's (still) in development - see http://www.unixzone.com/virus/
but neither AMaViS 0.2.0-pre6-clm-rl-8 nor AMaViS-Perl-5 currently
support scanning on a relay (it's still on my to-do list).
Two solutions:
a) use AMaViS 0.2.0-pre6 with two patches from my homepage
(rainer.w3.to) in combination with the SMTP store and forward proxy from
the Juniper Firewall
b) use AMaViS 0.2.0-pre6 with the outmail patch from Franz Ackerman,
see www.unix.cslab.tuwien.ac.at/~ackerman/outmail.html
I hope, AMaViS 0.2.0-pre6-clm-rl-9 comes with support for incoming and
outgoing virus scan.

Yes, libmilter it's available but marked as for future future release.
Several antivirus companies have announced sendmail integration.
